WHAT YOU’LL BE DOING

  • Obtain referrals from hospital social workers, physicians, or family members. Keep wait lists current. Be familiar with HENS process for referrals. Develop and maintain relationships with hospital social workers, staying current with their needs.
  • Function as the Health C enter marketing representative, providing tours of the facility and pertinent information regarding admissions. Make occasional off-site visits to hospitals and doctor’s offices.
  • Conduct admission interviews with the resident and/or responsible party. Obtain all required financial data, signatures, pertinent social information, medical information, IC D 10 codes attending physician information, and other pertinent and required documentation prior to admission, such as living will and durable power of attorney.
  • Review admission related paperwork with the resident and or/responsible party including Medicare Part A coverage, non-coverage procedures, and all HMO and managed care insurance. Verify available coverage. Collect copy of all insurance cards and confirm coverage with each insurance carrier.
  • Complete pre-certification prior to or immediately after admission. Coordinate all information from initial authorization with the MDS coordinator or billing staff.
  • Process and notify insurance companies for pre-certifications according to Medicare regulations before residents arrive at Health Center.
  • Maintain census in the Health C enter, tracking discharges and calling hospitals for admissions.
  • Track Life Care resident statistics: permanent assignments, and temporary assignments.
  • Coordinate utilization review for all new admissions covered by Medicare Part A.
  • Assist in coordinating admissions/transfers/discharges with other departments.
  • Conduct tours of the Health C enter for prospective residents, family members and/or guests. Marketing for Health C enter, update brochures, attend marketing events, and develop relationship with hospital discharge planners.

QUALIFICATIONS

  • Currently licensed as a Registered Nurse in Ohio.
  • Bachelors degree preferred.
  • Must have solid working knowledge of Medicare Part A, private insurances, managed care and certification procedures.
  • Must possess excellent written, oral and interpersonal skills, as well as the ability to juggle multiple priorities.
  • One to two years experience working in a long-term care facility preferred.
