As part of our LifeCare promise, Friendship Village of Dublin supports Residents in using all available resources in our premier senior living community and the central Ohio medical community.

A Licensed Social Worker and Experienced Senior Advocate

Our current Navigator, Ava Eiselle, MSW, LISW, has years of experience working with older adults throughout their life’s journey.
Since receiving her Bachelor’s and Master’s of Social Work degrees with a Specialization in Aging from The Ohio State University, she has advocated for older adults in community, residential, hospital and policy-setting environments.
As Navigator, Ava builds relationships, solves problems, and locates resources to help Residents learn about options and make confident, informed decisions about their care. She supports residents by coordinating care and linking them to resources.
The Navigator also is the “Go To” staff person to assist each Resident or spouse with decisions about how to deal with health changes.
“I’m here to provide as much support as each Resident needs and wishes,” she said. “I encourage any Resident who has an unmet need to contact me. Together, we will explore options and solutions.”
Coordinating Care and Providing Links to Resources
The Health and Wellness Navigator connects residents to a wide variety of programs and services available within our community, including:
- Central Ohio Primary Care
- Rehabilitation Services
- Home Care Services
- Health and Wellness Programming
- Podiatry and Audiology Services
- Medical Transportation
- Pharmacy Delivery
The Navigator also connects Residents to resources outside our community, including:
- Home Health Services
- Insurance Consultation (such as for Medicare and long-term care insurance)
- Psychological Services
- Medical Equipment Providers (such as for walkers and shower chairs)
- Legal Assistance (such as for living wills and advanced directives)
- Hospice
Additional Support in Independent Living
Needs often change with age. When a Resident needs additional support, the Navigator can discuss the barriers to living well in Independent Living and arrange resources that can improve success.
For example, our Home Care Services can bring high-quality, person-centered care right into a Resident’s Apartment, Flat or Villa.
Transitioning to a Higher Level of Care

When a Resident is ready to explore transitioning to a higher level of care, the Navigator can discuss what to expect and how the higher level of care differs from the current care setting.
She can also arrange a tour of Waterford Place Assisted Living and Alderwood Health & Rehab and assist with the transition process.
Arranging a Temporary Stay in a Higher Level of Care
If a Resident requires a temporary stay at Alderwood Health & Rehab, the Navigator will:
- Meet with the Resident before the transition,
- Continue to visit the Resident as a familiar face and Resident advocate,
- Participate in all Resident care conferences,
- Help develop the plan for the Resident’s return to Independent Living, and
- Smooth the Resident’s transition back home.
Coordinating a Planned Surgery or Procedure
If a Resident is planning a surgery or off-campus medical procedure, the Navigator will assist in coordinating their care to ensure a successful return to Friendship Village of Dublin.
Transitioning Back Home from a Higher Level of Care
When a Resident is returning to Independent Living from Alderwood Health & Rehab or a hospital, the Navigator will collaborate with the admissions coordinator to determine the correct placement for the Resident based on their individual circumstances.
The Health and Wellness Navigator will work with the Resident and their support system to ensure and monitor:
- Coordinating needed Home Care Services,
- Scheduling needed therapy services,
- Picking up prescriptions,
- Scheduling doctors’ appointments for follow-up care,
- Ordering needed equipment, and
- That the Resident’s other needs are met.
